Care Management
Care Coordination
Care coordination is the bulk of care management. It may include accompanying a client to appointments with other care providers, coordinating transportation, and helping to submit releases of information. She follows up on prior authorizations for medications, confirms insurance benefits, organizes medical and non-medical information, responds to emails, calls, and text messages to facilitate communication among involved parties, including facility staff, other providers, and concerned others approved by the client. Additionally, this may include submitting documentation and writing reports to obtain appropriate handicap benefits and equipment as needed.
Psycho-Social Support
A broad term referring to giving integrated support to a client and/or their concerned others. Stress can be mitigated through guided problem-solving and/or exploring options to reduce the negative impact of stressors.
Psychotherapy Services
Usually billable to insurance, these services offer a more in-depth, exploration of intrapersonal and interpersonal sources of distress. Due to the potential for conflicts of interest, these services are outsourced to other providers for clients receiving care management services from Cheyenne McCarter.
Request Services
All services are charged at $150 per hour.
Cheyenne chooses to disclose her fee out of respect for transparency and clarity in a complex web of care options. All services that require time, this includes travel time, are billed at the same rate.
Services offered that are not reimbursable by insurance include, but are not limited to: home visits, assessing needs and accessing resources, coordination of care with other authorized professionals and loved ones, telephone and email communications, reviewing records, writing reports, and providing ongoing assessment of needs as conditions change.
